What happens is, that various CD-Writing software and Windows itself adds callbacks to the IOP, but in Micosoft's short vision, they only have reserved 6 empty slots (and some of them are already used by Windows itself).
Most software doesn't check, if the table is full and crashes badly (WinOnCD 3.6, Nero and CloneCD.vxd).
This happens more often, if you have a device using Event Notification (Toshiba IDE DVD-ROMs), as Windows will occupy one Slot more (why? I don't know).
If you have WinOnCD and Nero installed on a machine with an IDE Toshiba DVD, and you have not deleted SCSI1HLP.VXD of Windows (using another slot), Windows, Nero and WinONCD already have written 7 (!!!) entries in this table, 1 over the end boundary. It is just luck, that the system is still alive, as the value behind this table isn't used for CD-ROMs. Now comes CloneCD.vxd (as evil as the others) and tries to write the 8th entry, overriding the IOR which follows this structure. This causes a big *kaboom*.
